"Determinants of the primary health care infrastructure in district cities of the Silesian Voivodeship under the SARS-CoV-2 pandemic"

Abstract

Local government units, especially urban municipalities, make a significant contribution to ensuring health protection to their inhabitants. This became particularly visible when family medicine practitioners used to help to prevent and treat the coronavirus. This paper sets out to explore density of the primary health care facilities, by means of ranking them, in 19 towns of the Silesian Voivodeship (so-called objects), in the shadow of the SARS-CoV-2 pandemic. The study was conducted in two periods: in 2020 (COVID-19 identification phase) and 2022 (COVID-19 adaptation stage). Within the framework of data comparative analysis, the standardized sums method was used, based upon normalization techniques like stimulants and destimulants standardization, quotients transformations and unitarization, associated by a Hellwig's taxonomic measure of development. Following the research hypotheses presented in the publication- H1: the network of medical centres (PYP) (variable Y) at the turn of 2020 to 2022 is influenced by three factors (Xi components): the number of nursery schools per 1 km2 (PRP), the amount of flats completed per 1 km2 (NMP) and the length of the gas grid in kms per 1 thousand inhabitants (ŚGM); H2: there are no significant differences between the Y and Xi components over the time horizon given; H3: a relatively high accuracy of individual tools for positioning the primary health care supply in Upper Silesia in 2020 and 2022 is confirmed, pointing at three dominant district cities in 2022 (abbreviated by SWI, KCE and CHO). Summing up the results of the city rankings, economic situation of the medical infrastructure improved mainly in two cases (object with the SSN code) (increase by +2 positions). In two of them, their position worsened the most (municipal units with the MYS and TCH abbreviations) (decrease by -2 places).
